Live Healthy South Carolina ― which is a statewide collaboration between the S.C. Department of Health and Environmental Control (DHEC), the Alliance for a Healthier South Carolina, and others ― is asking for your help getting the word out about a short 10-minute community health survey. The purpose of the survey is to help us better identify the resources our communities need to help improve the health of all our residents, both physically and mentally.
This survey is voluntary. We will not be asking for participants’ names. We will ask questions such as their county, age, race, gender, and occupation. Other questions will focus on the quality of life in an individual’s neighborhood and what healthcare needs are most important to them. Survey participants can choose to answer or not answer as many questions as they would like.
